For this design I use heat transfer vinyl and an industrial heat press at ~305 degrees F for print that's vibrant and longlasting, but still flexible and breathable. Either Gildan or Hanes brand 100% cotton tees depending on availability. The shirts are not pre-shrunk, but in my experience washing and drying them at high heat, they really don't shrink very much anyway.
